Schedule Your Initial Visit

Once enrolled, you’ll be able to schedule your initial comprehensive appointment where we establish care, review your history, and build a personalized plan.

Membership DOES NOT Include

  • Emergency care

  • Hospital services

  • Non-covered Administrative Services (i.e. Disability/FMLA, prior authorizations)

Enrollment: A one-time, non-refundable $150 onboarding fee per patient applies at sign-up and includes initial registration, medical record setup, and onboarding into the Practice.
Membership includes an initial 3-month minimum commitment. If membership is canceled before the end of that period, any refund is subject to recalculation using the standard monthly rate.
